Job Description

The Community Health Worker is responsible for providing outreach services from the site to the community, including home visits, symptomatic screening for households, and referrals to health facilities for further management. Additional responsibilities include education, information, counselling, and prevention.

YOU WILL BE REQUIRED TO:

  • Conduct TB screening in households and community settings.
  • Conduct TB testing and ensure receipt of results and linkage to treatment for people who tested positive 
  • Identify individuals who are HIV-positive and or diagnosed with TB but not enrolled in medical care through referrals, missed appointment follow-ups, and outreach activities.
  • Build trust and confidence in patients regarding healthcare services.
  • Provide accurate information about living with TB, HIV, preventing transmission, the benefits and challenges of treatment, and available services.
  • Assist patients with enrolment or re-engagement in medical care.
  • Support patients throughout their care journey to enhance education and self-advocacy.
  • Help patients build confidence in participating in TB and HIV treatment.
  • Collaborate with case managers and clinical staff to address patients' broader healthcare needs.
  • Co-facilitate support groups and link patients to psychosocial support.
  • Ensure compliance with programme protocols, including patient confidentiality.
  • Perform additional duties as assigned.
Job Requirements

Education 

  • Grade 12.
  • HIV/AIDS Testing and Counselling Certificate.

Advantageous

  • Report Writing.
  • Proficiency in Computer and Software Skills.

Experience 

  • A minimum of 6 months related experience OR one-year related experience and/or training OR an equivalent combination of education and experience.
